Members of the Board serve staggered four-year terms and they are appointed by the Mayor and by the County Commissioners "in proportion that the city's population is to the total county population." In 1976 three members were appointed by the Commissioners and four by the Mayor. Requirements for the composition of the board are as follows, 1) all must be residents of the county; 2) not more than four may be of the same political party; :3) three members must be licensed physicians (one may be a licensed veterinarian instead). The Board of Health must hold annual and quarterly meetings, must submit an annual budget to the Allen County Commissioners and to the Allen County Council and must provide offices for the Health Department. The Board of Health appoints a full-time Health Officer subject to the approval of the State Board of Health. The Health Officer, who must be a licensed physician, acts as the executive officer of the department and serves as the secretary of the Board. The Health Officer serves a four-year term. In addition to the Board of Health there exists a five-member Health Boar-d Commission. Members of the Health Board Commission serve staggered four-year terms. They are appointed as follows: two members are appointed by the County Commissioners; two members are appointed by the M:tyor of Fort Wayne; one is selected by the other four members. The sole function of this commission is to establish rules and regulations for the employment, rating, and promotion of the health department employees at or above the rank of sanitary officer, with the exception of the Health Officer. The Department of Public Health is responsible for promoting good health to the citizenry of the City and County and for collecting records. The work is carried out by several separate divisons: DIVISION OF VITAL STATISTICS: The Vital Statistics division is responsible for recording all births and deaths in the county and for issuing certified copies of these. Adoption records are also maintained here. Birth records are available from 1882 to the present and death records from1870. Certified copies of records may be purchased for a small processing fee. DIVISION OF FOOD AND ENVIRONMENTAL CONTROL: This division is responsible for inspecting and licensing all food establishments and food markets in the City and County. The Environmental Control Program makes sanitary inspections of nursing homes, motels, trailer courts, public buildings, and swimming pools and arranges for proper disposal of garbage, trash, and sewage. DIVISION OF PAROCHIALSCHOOLNURSING: Medical, dental, and health education is provided to students in the elementary grades of parochial schools. (A similar service is provided to public school students by the various boards of education.) Parents are notified of health problems and of care required. CLINIC DIVISION: This division is responsible for the control of communicable diseases, including diagnosis and treatment of venera I diseases. In addition, the division maintains records of all communicable diseases and reports this information to the State Board of Health. In 1969, the U.S. Public Health Services designated the Board of 30
